For Clinics/ Organizations /Doctors

Referring a client is easy! If you are a doctor, lawyer, or medical professional looking to refer a client over to our clinic for mental health support, all you need to do is complete the referral form below, and email it to us at admin@vaughanpsychotherapist.com or by Fax (905) 225-1855. A member of our team will be sure to reach out to them and provide you a follow up to ensure continuity of care.
